Unsullied Hearts are usually found on V Blood Carriers or other named enemies in the world once you start defeating enemies upwards of level 25 to 30. You can get this particular blood essence by finding Unsullied Hearts and then placing 4 of them in a Blood Press. The following journal quest is called “Servants” and requires you to build one of your newly unlocked Servant Coffins, which require 16 planks, 8 copper ingots, and 1 Greater Blood Essence.
